PHR
PHR stands for personal health record, a person- or family-managed set of health information kept for reference and care coordination.
What a PHR is
A PHR, or personal health record, is a set of health information organised by the person or family for their own use. In a parenting context, it usually means the copies and notes parents keep about vaccines, doctor visits, prescriptions, growth, and emergency details so that information is easy to retrieve when needed.
Parents often create one because hospitals, schools, and caregivers ask for the same information in different contexts. A PHR gives that material one dependable home.
How a PHR differs from official systems
A PHR is not the same thing as a hospital record, an insurance database, or an official digital-health account such as ABHA. Those systems serve their own formal purposes. A PHR is the family’s working layer: the version that helps parents stay organised between appointments, schools, travel, and caregiving handovers.
What parents usually keep in one
A good PHR contains the documents that matter in everyday life: vaccine records, prescriptions, lab reports, discharge summaries, growth measurements, and a quick emergency summary. It may also include parent notes, but those should be clearly labelled so they are not mistaken for clinician-issued records. The guide on how to organise your child’s medical records shows what that structure looks like in practice.
Why privacy still matters
Because a PHR is parent-managed, its usefulness depends on privacy and clarity. Sensitive health data should be stored carefully, access should be reviewed when caregivers change, and the family should still keep official records where they belong. The value of a PHR is not that it replaces formal systems. The value is that it helps parents coordinate real life more calmly.
Done well, it reduces repeated searching and helps both parents work from the same current set of records.
